13 week benefit duration
Guarantee Issue for groups of 5 or more with 100% participation, 75% if employees contribute towards their premium. Long Term Disability Program 60% of monthly earnings to a maximum of $7,500 monthly benefit
90 day Elimination Period
Benefits payable to age 65
5 year own occupation/any occupation thereafter
